If
someone is injured on your property:
- First,
show your concern. If the injured person needs
medical attention, call 911 or the number for
other emergency agencies in your area.
- Documenting
the incident is critical. Obtain the injured
partys name, address and phone number(s).

If there were any witnesses to the incident,
include this on your report. Be sure to get
the name/address/phone number of any witness.
- Do
not admit responsibility or liability. Show
your concern, but do not admit liability under
any circumstances.
- Take
pictures (if applicable).
Contact
our office as soon as possible after the loss
occurs. Our Claims Manager will take your
information and report the incident to the insurance
claims department. We will also contact any injured
persons to see how they are doing and to assess
their condition.

you contact us on a claim. Neither this document nor any
information you input will be secure unless you
do the following:
1.
Save the document to your computer before you
fill it out.
2. Close your browser.
3. Open the form from the location you saved it
to on your computer.
4. You can now complete the form and print it
or save and email it to us without security concerns.
